Client release form and background questionnaire as an RTF file that can be opened in any text editor or in Word or other word processing suites. This questionnaire is invaluable for determining whether the client should go to a doctor. The SCM does not replace regular medical care and if your client has a medical problem and hasn’t been to the doctor, please send them. If necessary, please tell them to go the doctor before having you work on them. I have told a few potential clients that I will not work on them _until_ they have seen the doctor. This questionnaire also gives you a good indication whether the SCM is suitable for the client. A client that has multiple issues, been to many doctors or therapists, has been modality hopping is usually a good candidate for the SCM.
